Clinical pharmacists are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach. Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.
FCPs are primarily physiotherapists working at an advanced clinical level who can provide a streamlined and cost-effective service in primary care by promoting self-management, improving patient care and reducing GP workload.
The podiatry service provides a range of specialist support and advice relating to the care of feet. They treat people whose feet are at high risk of developing serious foot complications such as inflammation, infection, ulceration and amputation.
The team offer 1:1, group and in some cases family/systemic support to patients aged 15 years upward who are struggling with mental health issues and who do not meet the eligibility criteria for Community Mental Health Team/Child and Adolescent Mental Health Service and who are not appropriate for/not working with Steps2 Wellbeing.
The social prescribing team have been working for Christchurch Primary Care Network for 3 years. In that time they have worked with many people in the local area offering support with increasing their social activities, signposting for advice with benefits, housing, advocacy, domestic abuse, physical health activities, employment and volunteering. They work closely with community groups and charitable organisations who provide invaluable support to local people. The team have an extensive background working within nursing, occupational therapy, sports psychology and in the charitable sector.
The team liaise between health and social care to assist complex navigations for patients and their families. They bridge the gap and pull together multi agency working to avoid areas being missed/work duplicated. They can make initial referrals to Social Services and co-ordinate with them regards existing clients with a change of circumstance (eg, requiring a move to residential care)
By setting small goals to support people in improving their health and wellbeing the team hope to evoke and develop their skills, knowledge and confidence to enable behaviour change in them becoming active participants in their care, so that they can reach their self-identified health and wellbeing goals to manage their own health as effectively as possible.
