Privacy Policy

 1. Who we are

Christchurch Primary Care Network (PCN) is a collaboration of GP practices working together to deliver enhanced primary care services for the local population.

Christchurch PCN does not directly hold, store, or process patient identifiable data.
All patient records and personal data are held and managed by the individual GP practices within the PCN.

2. GP practices within Christchurch PCN

The following GP practices are members of Christchurch PCN. Each practice acts as an independent Data Controller for its registered patients:

  • Highcliffe Medical Centre
  • Christchurch Medical Practice
  • Stour Surgery
  • The Farmhouse Surgery

Each practice has its own privacy notice explaining how your personal data is collected, used, stored, and protected.

3. How your information is used within the PCN

While Christchurch PCN does not directly access or store patient medical records, GP practices may share limited and appropriate information where necessary to:

  • Deliver PCN services (e.g. Enhanced Access services, ARRS roles)
  • Coordinate and improve patient care across practices
  • Support population health management and service planning
  • Meet statutory, contractual, and regulatory obligations to NHS England

Any sharing of information is carried out lawfully, securely, and in accordance with:

  • UK General Data Protection Regulation (UK GDPR)
  • Data Protection Act 2018
  • NHS data protection and information governance guidance

4. Data controllers and responsibility

Each GP practice within Christchurch PCN remains the Data Controller for its patients’ personal data and is responsible for:

  • Ensuring data accuracy
  • Keeping information secure
  • Responding to Subject Access Requests
  • Managing patient rights under data protection law

Christchurch PCN supports collaborative working but does not make independent decisions about the processing of patient data.

5. Your data protection rights

Under data protection law, you have the right to:

  • Access your personal data
  • Request correction of inaccurate data
  • Request erasure (where applicable)
  • Restrict or object to certain types of processing

To exercise these rights, please contact your registered GP practice directly, as they hold your records.

6. Data sharing and third parties

Where third-party organisations are used to support or deliver PCN services, appropriate Data Sharing Agreements and Data Processing Contracts are in place to ensure personal data is handled securely and lawfully.
